Sir,

REPLY TO QUERY

With reference to your query, Ref. No. 31110/1.48B/2011/2580 dated on 11th September, 2013 which was handed over to me on 17th September, 2013 which says that necessary safety precautions were not taken before the commencement of the said work.

My submission is that the necessary precautions were taken before the commencement of this work.

On 9th August, 2013 at about 17:15 hours when our feeder tripped my manager Engr. (name) called Mr. (name) the operator at the TX station to grant us outage on our feeder to enable us tidy up (a location) 300KVA sub-station that was tested Okay by PC&M crew and this was done in the presence of Mr. (name) and the outage was confirmed by the operator at the TX. Thereafter we went and grounded the lines and these safety precautions gave Mr. (name)  the courage to climb up to work and it was at this point that I left the scene to attend to my sick mother after taking permission from my boss.

Moreover, the issue of not signing station guarantee at the TX station is because of the distance which is more than 70Km from (a place) and also considering the bad nature of the road which makes it difficult for us to go to TX station and sign the station guarantee (S.G) because it takes one almost 2hours to get to the TX station and the same time to get back to do the work in (name of place) with only one vehicle which ends up grounding other activities for the day in our office. It is based on these difficulties that the transmission (TX) station gives us outage on air when the need arises.

Sir, I therefore request that I should be exonerated from any blame since all necessary safety precautions were taken before I left.

Yours faithfully,

Signature
