ABSTRACT
The study was focused on the Role of Health Information Management in Primary Healthcare Delivery system: A case study of Abak Local Government Area, Akwa Ibom State. The research adopted a descriptive survey design and simple random sampling technique using simple percentage for data analysis.
The population is made up of 150 sample size. A set of questionnaire was used to obtain data and 130 copies were retrieved out of 150 copies administered. The finding revealed that Role of Health Information Management in Primary Healthcare Delivery System ensures healthcare facilities with the highest standard of data integrity and security.
Therefore the researcher on conclusion of the study recommended that there should be; employment of trained Health Information Managers at the grassroots level for proper registration and documentation of patient record or Health Information.

INTRODUCTION
Health Information Management (HIM) professionals play a vital role in the delivery of Healthcare in Nigeria through their focus on the collection, maintenance and use of quality data to support the information intensive and information reliant healthcare system. Health information reliant healthcare system.
Health information management professionals practice in an increasingly diverse set of roles in a wide variety of healthcare settings providers and payer organisations public and privates agencies, research, licensing, accreditation etc. and vendo organisation which provide health information tools product as well as consulting an out sourcing services. AHIMA (2008).
Health Information is a collection of clinical information pertaining to patient physical and mental health complied from different sources. Health records being a clinical scientific, administration and legal document relating to patient care in which recorded sufficient data written in sequence of events to justify the diagnosis and warrant treatment.
Health records contains demographic data, next of kin, and most of the following clinical surgical procedure and drug therapy, medical history, examination diagnosis; treatment. Result of investigation laboratory (e.g. biochemistry, hematology, pathology), imaging (e.g. plain films, scan).
Health information plays a major role in terms of patient care research, training of medical and other para-medical student and statistical report http/www.AHIMA.Org, (2003).It also used in the protection of hospital staff, making of issuance program and patient care evaluation.
